If you are seeking services for your child or are an agency seeking services for a client, please submit a request using the contact form below. Please include a brief description of the nature of the need for Occupational Therapy services. If your child or youth has been referred by another professional or agency, please include this in your request. Requests for services and the waiting list are reviewed on a weekly basis. The waiting list is dynamic and fluctuates, but generally ranges from between 4-8 months.
